Chronic disease sufferers ( Stroke , Cancer , respiratory problems , Diabetes and heart disease) that require long and short term home based care often have little time to put in place measures for their care after discharge. Coming Home has identified a need for support services to assist the patient, their family and their primary care giver from the point of discharge to their reintegration back into the family unit, their workplaces and society in general. It must be noted that this transition is often fraught with uncertainty and lack of knowledge about home based care, time required to care for the patient, limited finances and equipment to manage the patients care within the home. Coming Home also recognizes the value of educating the broader public on treatment and palliative care, especially in non-urban areas where basic and relevant information is either scarce or not easy to access.
